Your dental records, finally yours.
Face Dental Group is one of the first practices in the country to offer every patient a
Dental Passport — a secure record of your care that travels with you,
not one locked in our filing cabinet.

Dental Passport
Free for every patient · takes about two minutes · works on your

Read your own visit notes — in plain language
After your visit, Prof. Dr. Wael Att’s AI scribe writes up what happened in words you can
actually use: what we found, what we did, and what comes next. It lands
in
your passport automatically, usually before you’re home.
Most patients have never once seen their own clinical notes.
Ours read theirs on the drive home.
What’s in your passport
Visit recaps & clinical notes
A plain-language write-up of every appointment, added automatically.
Health history — entered once
Fill out your medical history one time. Never re-fill a clipboard form
again.
Insurance on file
Your coverage details, ready whenever you or a new office needs them.
Implants & appliances
A permanent record of exactly what’s in your mouth — make, model, and date.
Documents & signatures
Consent forms, treatment plans, and paperwork — all in one place.
A complete access log
See every time your records were viewed, by whom, and when. Every access,
ever.
Moving? Seeing a specialist? Your records go with you.
Forward your records yourself in two taps — no release forms, no fax machines, no waiting
on hold. You can add family members to one account, too, so the whole household’s records
live in one place.
We built it this way on purpose. We’d rather earn your next visit than hold your
records hostage.
Frequently asked questions
Is the Dental Passport free?
Yes. Every Face Dental Group patient gets a Dental Passport at no cost. There is nothing to
install — it works in your phone’s browser.
Is my Dental Passport secure?
Your records are encrypted, and your passport keeps a complete access log — you can see
every time your records were viewed, by whom, and when. You control who gets access,
and
you can revoke it at any time.
What happens if I move or switch dentists?
Your passport goes with you. Forward your records to a new dentist or specialist
yourself
in two taps — no release forms, no faxes, no waiting on hold. Your records belong to
